Tuition Payment Plan: By signing the Enrollment Agreement, you represent and warrant that You are the authorized parent or legal guardian of the student and You hereby authorize the student to be provided access to Graham Private Christian Online Learning. By signing this agreement, you are verifying that You have read this entire Agreement and have explained its entire contents to the student in age appropriate language. Further, You understand and agree that both You and the student are bound by the terms of this Agreement and agree that access to Graham Private Christian Online Learning is contingent upon You and the student adhering to the terms of this Agreement and that failure to do so may result in no longer having access to the service. Welcome to the family of distance learners at Graham Private Christian Online Learning. This Enrollment Agreement is intended to make the School’s relationship with You clear, and to set forth some of the School’s policies that will govern that relationship. If You have any questions or concerns that are not answered or addressed in this Enrollment Agreement, please call the Customer Service Department for further information. Graham Private Christian Online Learning partners with you to provide educational services and support for each student. This agreement ("Agreement") is between Graham Private Christian Online Learning ("School") and the Parent/Guardian/Student(s) Party ("You", "Customer") of any Graham Private Christian Online Learning’s products and services ("Service"). This Agreement governs all Services. By activating the Service, you acknowledge that You have read and understood, and You agree, to the terms of this Agreement. Graham Private Christian Online Learning may change the terms of the Agreement from time to time. Enrollment Agreement: Tuition fees are based on either per-course pricing, one semester (6 courses) or full year (12 Courses). Additional courses may be taken but must be purchased in addition to the payment plan. Tuition includes curriculum, teacher and administrative services, college/career counseling options, education support service, ongoing customer service and maintenance of student records, evaluations and transcripts, etc. Tuition or other fees may change due to enrollment specials or increase in fees. Payment Plan: Graham Private Christian Online Learning families may pay in full at enrollment or take advantage of the School’s payment program of monthly payments over 2 months enrollment per course. This fee is valued at a fixed amount per course. Paid in full or paid by monthly payments divided into 2 . All payments are made from a credit card account or a direct debit from a bank account. A $35 fee shall apply for any insufficient funds. Since the Graham Private Christian Online Learning has reserved a space for the student, payment is due each month whether or not work has been submitted to the teacher. Enrollment is based on semesters. Students finishing a course or grade level in less time than planned are still required to pay the full tuition for the entire term. Final grades and/or certificates of completion will not be submitted until all outstanding charges are paid in full. Additional courses beyond standard enrollment (6 courses per semester or 12 courses per year) may be taken but must be purchased in addition to the payment plan. Graham Private Christian Online Learning reserves the right to suspend services if monthly payments are not made in a timely manner. A grace period of no more than 5 days shall be allowed. A reactivation fee of $250 will apply to all suspensions of service. If suspended, all tuition will be due to date and liable each month until a notice of withdrawal is received. Graham Private Christian Online Learning reserves the right to withhold issuing transcripts and/or diplomas for any student not in good financial standing with the School. Extension Fee (only if necessary) Due in full at time of extension: A student who has not completed his or her course of study within the applicable period following enrollment (semester or year) may request an extension of the completion date, if eligible. A fee shall apply to each extension granted at a rate of $200 per course per month past the course completion date. Final grades shall only be given for work completed within the applicable term, as may be extended. In some extreme cases a full re-enrollment may be necessary. Student Responsibility: Graham Private Christian Online Learning students are provided with ample opportunities, resources and instructional staff to succeed in this program. However, students still bear the weight of personally committing to their own academic success. Graham Private Christian Online Learning students are responsible for the following: Daily engaging in assigned academic studies (Daily engagement equates to attendance which is both mandatory and actively monitored). Completing all assigned tasks. Independently maintaining a successful studying environment. Asking for help when needed. Regularly communicating with assigned teacher(s) including a minimum of two forms of communication per week. Daily checking email correspondence and replying to all forms of communication in no less than 24 hours.

Keeping parents/guardians informed of progress, grades, and communication from the assigned teacher(s). Following School rules and policies. Upholding a strong moral ethic with regard to academic honesty; especially in regard to cheating or plagiarism. Parent Responsibility: To ensure a successful and rewarding distance learning program, each parent is an essential part of his or her child’s education. Nationwide studies show that students who receive regular support from their parents do better in school. Parents are responsible for the following: Contact Your student’s teacher(s) on a regular basis. Provide student(s) with access to an internet connected computer and a telephone. Provide a suitable place for the student to study and help develop a workable schedule to complete assignments. Have funds available for monthly payments. Notify the School of any address, phone number and email changes or leave of absence. Notify the School if an extension or re-enrollment is needed to finish a course or grade level. Notify the School with a “Written Notification of Withdrawal” to withdraw from Graham Private Christian Online Learning or drop a course.

Review and discuss online grade postings regularly with student(s). What is Included: All enrollment packages include costs for curriculum, teacher and administrative services, college/career counseling options, education support service, ongoing customer service and maintenance of student records, evaluations and transcripts, etc. What is Not Included: A one-time, non-refundable, enrollment fee of $250 will be added to all initial first-time enrollments to cover costs associated with enrollment. In addition, some classes may require additional materials such as art supplies or novels (if not available from a local library or online source). Courses requiring additional supplies will be noted and should be reviewed prior to enrollment to ensure adequate access to learning materials. Judgment on the award rendered by the arbitrators must be entered in the state and federal courts of Saint Lucie County, Florida. The arbitrator shall have no authority to award punitive damages. Customer acknowledges that this arbitration provision constitutes a waiver of any right to a jury trial. Limitation of Liability: The Graham Private Christian Online Learning shall not be liable for any failure to provide the Service, at any time or from time to time, or any degradation of text, image or transmission quality that is caused by any of the following: • act or omission of an underlying carrier or third party; • equipment, software, network or facility failure; • equipment, software, network or facility upgrade or modification; • force majeure events such as (but not limited to) acts of god; strikes; fire; war; riot; government actions; • equipment, software, network or facility shortage; • equipment, software or facility relocation; • service, equipment, software, network or facility failure caused by the loss of power to Customer; or • any other cause that is beyond the School's control, including without limitation the failure of an incoming or outgoing call to be connected or completed, or degradation of voice quality and fax transmission quality. The Graham Private Christian Online Learning's liability for any failure or mistake shall in no event exceed the fees student has paid the Graham Private Christian Online Learning (minus the enrollment fee of $150). The Graham Private Christian Online Learning is not liable for incidental or consequential damages of any type. All services provided by the Graham Private Christian Online Learning are provided “as is.” The Graham Private Christian Online Learning does not warrant that the service will be uninterrupted or error-free, or service will be corrected. Attendance: Graham Private Christian Online Learning will only maintain daily attendance records for full-time students. Students taking single courses or attending only part-time will not be counted in the school’s attendance records. The responsibility for part-time student attendance compliance with local/state laws will lie solely with the parent/guardian(s) of the part-time student and not with Graham Private Christian Online Learning. In addition, the great variance in attendance laws from state to state and out of state necessitate that Graham Private Christian Online Learning will not be responsible for ensuring compliance with local attendance laws for full-time and/or part-time students. By enrolling in Graham Private Christian Online Learning, the student is enrolling in a Florida private school. The responsibility of compliance with local attendance laws lies solely with the parent/guardian(s). Severability: If any provision of this Agreement is held to be invalid, void, or illegal by a court or arbitrator of competent jurisdiction, the same shall be deemed severable from the remainder of this Agreement and shall in no way affect, impair or invalidate the remaining provisions herein. If any provision shall be deemed invalid due to its scope or breadth, such provision shall be deemed valid to the extent of the scope or breadth permitted by law. Participation in Graham Private Christian Online Learning is optional. Should You allow Your student to participate in Graham Private Christian Online Learning, You and the Student must abide by this Agreement. This agreement is in effect upon the receipt of the registration and tuition payment. I have read and agree with the policies of this agreement.
